When calling to schedule your first appointment with us, please keep in mind we will be requesting the following information:
If you have had x-rays taken in the past year from another dentist, you are welcome to bring them to your first appointment or have your previous dental office e-mail them to us at shp3240@gmail.com.

- What is your phone number?
- What is the purpose of your visit? Are you coming in due to pain or for a check-up / cleaning?
- If you are coming in for a cleaning appointment by one of our registered hygienists, you will first need to be examined by Dr. Ha. If deep cleaning is required, the procedure must be completed prior to your basic cleaning.
- Do you have insurance?
- If yes, what is your ID number or social security number? What is your group policy number? When is your birthday?
- Who were you referred by?

We accept all major dental PPO plans. We do not accept HMO or Medicare / Medi-Cal.

Our patient forms are available online for your convenience. If you would like to complete them at home, saving you waiting time in our office, please download the three forms below. You may print out the forms and bring them to your first appointment. Otherwise, you may complete them at our office on the day of your appointment.
